CIRCULAR
PROVIDING FORMS OF INVESTMENT MONITORING AND EVALUATION REPORTS
THE MINISTRY OF PLANNING AND INVESTMENT
Pursuant to Law No. 38/2009/QH12 Amending and Supplementing a Number of Articles of Laws Concerning Capital Construction Investment;
Pursuant to Investment Law No. 59/2005/QH11;
Pursuant to the Government's Decree No. 113/2009/ND-CP of December 15, 2009. on investment monitoring and evaluation;
Pursuant to the Government's Decree No. II6/2008/ND-CP of November 14, 2008, defining the functions, tasks, powers and organizational structure of the Ministry of Planning and Investment;
The Ministry of Planning and Investment provides forms of investment monitoring and evaluation reports as follows:
Article 1. Scope of regulation and subjects of application
1. Scope of regulation: This Circular provides in detail forms of investment monitoring and evaluation reports specified in the Government's Decree No. 113/2009/ND-CP of December 15, 2009, on investment monitoring and evaluation.
2. This Circular applies to organizations and individuals involved in the elaboration of investment monitoring and evaluation reports.
Article 2. Forms of investment monitoring and evaluation reports
1. Forms of investment monitoring and evaluation reports issued together with this Circular include:
a/ Form No. 1: biannual and annual report on investment monitoring and evaluation;
b/ Form No. 2: monthly report on the implementation of investment projects with 30% or more state capital:
c/ Form No. 3: quarterly, biannual and annual report on monitoring and evaluating the implementation of investment projects with 30% or more state capital:
d/ Form No. 4: monitoring and evaluation report upon adjustment of investment projects with 30% or more state capital;
e/ Form No. 5: monitoring and evaluation report upon termination of investment projects with 30% or more state capital:
f/ Form No. 6: biannual and annual report on monitoring and evaluating the implementation of investment projects funded with other sources of capital.
2. In these forms, the contents in italic are for instructive and illustrative purposes which may be concretized by report makers to suit the practical situation.
Article 3. Organization of implementation
1. This Circular takes effect on July 17, 2010.
2. Ministries, ministerial-level agencies, government-attached agencies, other central agencies. People's Committees at all levels and concerned organizations and individuals should report problems arising in the course of implementation to the Ministry of Planning and Investment for timely consideration and settlement.-
